The size of the map doesn't really matter, but the amount of things you have packed into them. I beat the game before the DLC came out so I went right to new game plus. Now I have all the DLC and want to play it but the level for the missions are now 62 and 64 to start. So I essentially need to beat the game again before I can do the DLC. I'd suggest doing the DLC first. At least for me it feels like a bit of slog now just to play the DLC.

You have to restart and get all your shit again. I noticed the game of the year ones had pegi 18 ratings while.the complete editions had M ratings.
If you feel like playing the base game before touching the DLC, I'd definitely recommend NG+. On the other hand, not replaying the game shouldn't be too big of an issue. It'll just be more difficult to buy certain things, not a real problem. Do whatever you like, but keep in mind that doing NG+ has a slight advantage.

Main campaign completed, will come back to DLC. Played on Blood and Broken Bones Difficulty (don't really remember the exact minutes time) Mixed with some trophy hunting, so game time without that turns to ~ 50h. Lots of side quests done in main game. Havenât touched expansions.
